Your CSV file should have below format. If you want to add items with child items in package, mention “Yes” in Include Child column else “No”.

There is Sitecore Support Toolbox module that does similar things for 2 years already 🙂 it can also include all references like templates, presentation items and files, media, workflows etc.

Yeah Alen, I know it's good tool.
But this module is different, it allows you to create package based on random item list.
You only need to provide item list in form of CSV file and it will create package for you.
TDS is your friend.
Yes, TDS is great but expensive also. This free tool allows for developers who have not invest in TDS License to generate packages automatically:)
